Multi-level checkboxes or radio buttons

Does the option exist to create more than 1 level of radio or checkboxes?

Example:

1. Select your affiliation:
   [ ] As a researcher
   [ ] I need high performance storage
   [ ] I need storage for home directories

   [ ] As an administrator
   [ ] I need storage for applications
   [ ] I need storage for files

I am able to create facets that have multiple levels, but unable to get them to display on my page.

[ ] - represents either a checkbox or radio button...
