Fix or delete broken links to host web sites

The following links to host web sites are currently broken. We should
get in touch with the organizers to see if that's something that can
be fixed or whether the sites a definitely down. In the later case, we
should remove those links.

• Tianjin'07,
• Vancouver'04
• Melbourne'02
• Barcelona'99
• Toronto'98

I removed the link from Toronto'98 because I am confident that the machine is gone. I'll ask Nantel if the contents of the site might exist in a backup.

I look at the comments from Bridget above and I am not sure of those were ever transferred to the appropriate pages.
