Add links to the proceedings

The conference pages now include a link to the proceedings if appropriate metadata (title + url) are specified. Thanks to information harvested by Christian, the metadata is provided for about two third of the conferences, with one caveat: the Discrete Mathematics volumes don't contain
all the papers, only a selection. There remains also to fill in the information for the remaining conferences:

I just stumbled over this; sorry if this is common knowledge: the proceedings for 1992 are hosted at http://lacim.uqam.ca/en/les-parutions/ and can be found at http://lacim.uqam.ca/wp-content/uploads/Publications/11.pdf -- although it took a rather long time downloading.
