Move bootstrap.js files from bottom of page to header #3

Changes to survey1.html.
I had to do some intrusive updates into REdcap classes to allow for javasript includes at the end of the file instead of in the header. I'd like to avoid this as it will be trouble when we need to update Redcap later. Can you please take an hour or so to look at putting the bootstrap js into the header instead of in the footer. I tried it and the popover stopped working correctly.
