Naming a lib method param creates a lib mapping

ex: naming the param of an Object::equals implementation creates java/lang/Object.mapping

probably introduced in #306, specifically because it:

removes java/lang/Object's explicit exclusion from InheritanceIndex

passes the combined index to EntryRemapper instead of the main jar index

I think this can be solved by checking if a param's parent is a lib method.
We should be able to resolve an entry up to its deepest non-lib ancestor.
