Missing all_product_slates.csv file #11

The latest version does not have an all_product_slates.csv file available. Furthermore, when attempting to workaround this by using an all_product_slates.csv file from a previous release, a "ValueError: 'Field Name D" appears. Removing the "Field Name D" placeholder(?) from the opem_inputs.csv means the ValueError does not pop up, but there still doesn't seem to be any results in the opem_outputs.csv. Also for clarification purposes, isn't the setup.py script run to use the opem_inputs.csv and product_slates.csv files together to generate a populated opem_outputs.csv file?
