Permit user to add fields to Create Disbursements interface

At present, the Create Disbursements LWC displays four fields at the top of the screen —

1. Recipient Name
2. Awarded Amount
3. Paid Disbursements
4. Available for Disbursement

Ideally, a user would be able to choose fields by editing the field set. For instance, Term Start Date would be a helpful addition, as the date is critical for the "First Date" field — but the "First Date" field defaults to TODAY().
