Reference Discord channel by ID (and name) instead of only name

When you have two channels with exactly the same description, then the bot may select the wrong channel to use. This can be the case when two CTFs are running within one Discord server (with the same challenges). We can fix this by using the task ID as part of the identifier to make it properly unique.
