Add flu vaccine claim page and implement questions for it #22
Description
The flu vaccine service page should be added and should have the following questions:
- Where is the vaccine to be received?
- Date of birth
- When is it to be received?
- Have you received at least two flu vaccines previously?
- How many flu vaccines have you received this year?
