Documentation: include the order of file creation in the instructions #69

I guess the general idea is that one first writes human language and then code, but in practice if someone wants to adopt entangled in an existing project, they may try to wrap human text around existing scripts. So in "Setting up a Literate project" it should be mentioned that one first writes the name of the script file in the markdown and entangled creates that file. Otherwise it seems quite intuitive to create the file on one's own.
