when adding user, it returns to stage1

whenever i try to add a user, when doing the setup, it makes me go back to the first stage (requirements).

I have been looking at the code and i'm not able to figure it out.

the only thing, is that the form, that has that button "add user" the action is empty, i think that is the issue.
