Document error format

We should document (and probably test thoroughly) the errors returned by the validator so projects using this as a module don't have to play a guessing game when handling error conditions.

Part of that effort should include that our current errors, all return at least a reliable base set of information that can be displaying in the validator service. See #10 (comment) for an example of where we might not be doing the right thing.
