Don't show empty fields

First, great job on that npm!
Some forms store a lot of fields and many of them are often empty.
It would be great to only show the fields for those how have an actual value in 1 Password.
I'd be happy to submit a pull request, but I noticed that you haven't responded to the 2 currently open ones. Are you still maintaining this project?
