Author address and phone number text boxes needed

Hi there, would it be possible to get required text boxes for author address and phone number inserted into the APC plugin please, so that when enabled this requires an author selecting they can pay an APC to fill in their address and phone number, so that when the information then goes through to the individual sorting the APC they have these details already in there for invoicing. Thanks for your help.
