Google QR Code API Retired

Google have retired their QR code API. A quick and dirty fix is the following change to paul999/tfa/modules/otp.php

Change:
'TFA_QR_CODE' => 'https://chart.googleapis.com/chart?chs=200x200&chld=M|0&cht=qr&chl=' . $QR,
To:
'TFA_QR_CODE' => 'https://qrcode.tec-it.com/API/QRCode?backcolor=%23ffffff&size=small&quietzone=1&errorcorrection=H&data=' . $QR,
