Confusion of the Atmega firmware

I have read the README file and just can't work out if I need a chip with the Arduino boot loader installed or not!

It refers to a bootloader install first, but is this the "normal" atmega boot loader, that allows serial upload of the sketch? or what?

What is the correct procedure to put the sketch on the atmega328

You can use a chip with the normal bootloader but it is not required. What is required is to program the MEGA with the same fuse settings that the bootloader uses (16 MHz etc.) which I don't remember off the top of my head, but if you program the bootloader it programs the fuse settings as well.
