Adding an example questionnaire? #34

It's just an idea, I would like to have some opinions about: If you start a new questionnaire in the editor of sdaps_web, there is just an empty page. Should we add an example questionnaire, so people see, what sdaps is capable of? Debugging via e.g. selenium would also become a little bit easier?
