Folder Support

I'm happy to code this and provide a PR, however a pointer in the right direction would be really approached.

How would I be able to upload to a specific folder in Cloudinary?

If you want to save your files using folder you should set public_ids like 'test/test.jpg' and allow automated folders creation in your account settings in Cloudinary dashboard.
