Move currency symbol to a variable in index.php

Currently the `$` can be replaced with `R` in the [jquery.supportform.js](https://github.com/watchmanmonitoring/SupportAgreement/blob/master/sample-agreement-php/jquery.supportform.js) file and in the index.php

This should become a variable in index.php
