Added icon in .toc file.
Fixed showing placeholder under a top level header that has no direct descendents.
Add dummy entry below empty headers so it's clear why there are no factions shown.
Fixed issue post-migration that broke folding of the last two entries.
Fixed some issues with folding, and fixed bugs with toggling/displaying inactive reputations.
Fixed issue where folding a child category would hide reputations on the same level that are after it. Fixes bug #41.
Fixed issue with duplicate names for factions (Steamwheedle Cartel). Some loss of data for that faction might have occurred. Fixes #36.
Update Interface version
Update toc to use unified one.
