Based on PR #315

Chain of upstream PRs as of 2025-11-23

Simple form is required to be updated due to only gaining support for
Rails 5.1 in the late 3.X series, but we can safely go all the way to
4.1 and avoid future upgrades for a few patches.

Simple form is required to be updated due to only gaining support for
Rails 5.1 in the late 3.X series, but we can safely go all the way to

They now include more table information and the format is a little less
horizontal.
