Large overhaul of functionality:

• Removed VCF parsing (now handled by split-vep conversion to TSV in nf-cavalier)
• Remove IGV snapshots (now handled by igv-reports in nf-cavalier)
• Added Gencode to generate gene lists from linkage regions.
• Added Monarch Initiative OMIM cache to get OMIM diease names.
• Moved docker build to GitHub workflow, simpler container based on rocker/tidyverse
• Fix HGNC URL issues
