Description of changes

Adds the following instrument prompts and example notebooks:

• Summary of Care: Evaluates summaries of inpatient care. In addition to clinical notes, these summaries may reference other discrete clinical data such as labs & imaging results, medication administrations, and vital signs over time.
• Draft Appeal: Evaluates medical necessity justifications as they would be written in clinical denial appeal letters.
