Updated the plugin to align with the latest PAI hooks architecture: - Refactored `src/lib/paths.ts` to use `PAI_DIR` (default `~/.claude`) for consistent path resolution. - Added `src/lib/metadata-extraction.ts` for extracting agent instance metadata. - Updated `src/lib/logger.ts` to log events in the PAI standard JSONL format with PST timestamps and agent metadata. - Updated `src/index.ts` to use the `event` hook properly, removing legacy `chat.message` usage. - Implemented enhanced tab title updates based on user prompts. - Removed obsolete `src/lib/context-loader.ts` and `src/lib/notifier.ts`.
