docs: Add process flow research documentation #4

Analyzed the `src/index.ts` entry point and its dependencies to document the actual execution flow of the plugin. Created `docs/process-flow-research.md` detailing: - Initialization and hook registration. - Event processing loop (filtering, logging, terminal updates). - Tool execution logging and agent mapping. - Data persistence strategies. - Visual process flow using Mermaid. - Identification of unused modules (`context-loader.ts`, `notifier.ts`).
