Add Filament 5 and Livewire 4 support

Summary

• Adds support for Filament 5 and Livewire 4 to the package
• Updates CI workflow to test against Filament 5 with Livewire 4 on Laravel 12
• Maintains backward compatibility with Filament 4 and Livewire 3 on Laravel 11/12
• Excludes Filament 5 from Laravel 11 since Filament 5 requires Laravel 12+

Test Matrix

The CI now tests the following combinations:

• PHP 8.3/8.4 + Laravel 11 + Filament 4 + Livewire 3
• PHP 8.3/8.4 + Laravel 12 + Filament 4 + Livewire 3
• PHP 8.3/8.4 + Laravel 12 + Filament 5 + Livewire 4
