Move quality/stars to second line and add a large V grade element floated to the right that spans the full height of the title section.

- Fix textSecondary to neutral[500] for valid theme token - Combine quality, setter, and ascents on second line with dot separator
- Parse V grade from difficulty (e.g., "6a/V3" -> "V3") - Large grade display shows only V grade - Second line shows full grade before stars (e.g., "6a/V3 3.5★")
