PDF creation probably should be changed to have titles too

I think some things should be discussed with product team:

1. Which fields should be required (only title/only text/both) or is it okay to have at least one field filled in (current implementation)
2. Should providers be able to search by text if title is empty (for already created instructions)

This was already discussed in the Linear task.

Regarding the decisions: both instruction and title are optional, and search is performed only by title.
