http://flickholdr.com/${w}/${h}/fixie was not working anymore. I replaced it with holder.js as suggested in #43.
Removed references to http://fixiejs.com/ since the page doesn't offer the original content anymore. Updated the part about setImagePlaceholder to a working site for placeholder images.
