Looks good!
One small comment I have is about the annotations being a list. Other charts in this repo use dictionaries for annotations so I think it's worth doing the same here. Annotations are key/value pairs anyway so using dictionaries feels more natural for those.

It is based on what is currently working in internal Kubernetes clusters
