here's a proposed flowchart for how users navigate through and provide information, plus some notes on requirements and what I can quickly mine from the challenge site

In the flowchart, I think the collection of important info like (Name & Address) should go at the top right after the (Do you understand ?) question. And maybe we add a summary page at the end where they can see all the info in one single page and they put their digital signature to finalize the submission.
